BASIC LIFE INSURANCE

The City of Richardson purchases life insurance for its employees. The coverage amount is based on twice your annual salary (rounded to the nearest $1000) to a maximum of $50,000. This group life insurance is a term policy which does not carry any cash value. Upon termination or retirement, employees may convert their basic coverage into an individual policy.

The City of Richardson also purchases AD&D coverage for its employees. This coverage amount is based on twice your annual salary (rounded to the nearest $1000) to a maximum of $50,000.

SUPPLEMENTAL LIFE COVERAGE FOR YOU AND YOUR DEPENDENTS

New, full-time employees have 31 days from their date of hire to elect supplemental life coverage for themselves and their dependents. This coverage is in addition to life insurance that the City purchases for employees. Newly hired employees may opt for coverage up to $250,000 (guaranteed issue amount) and spouses up to $50,000 without medical underwriting. Any amounts elected above the guarantee issue requires completion of an Evidence of Insurability form. Minnesota Life will send you an email with a confidential link, and ask you to complete a questionnaire. If you are not immediately approved, additional information will be requested. If you do not have a City email and choose amounts over the guaranteed issue, Minnesota Life will mail you a questionnaire. Complete the forms and return them to Minnesota Life. Employees must purchase coverage for themselves to elect dependent coverage, which may not exceed the amount purchased for the employee.

BENEFICIARY INFORMATION

Your beneficiary for basic life insurance and supplemental life insurance (if elected) must be the same. Please complete the beneficiary form or contact Becky Murray (972.744.4005) or Chelsie Montgomery (972.744.4007).
