Forms available for download from the City of Richardson Municipal Court:

  • Driver Safety Course Application Form
    [Note: This form must be notarized and filed with the Richardson Municipal Court on or before your appearance due date. Your payment must accompany this form upon filing with the Court. Do NOT use the on-line Internet payment system for payment of your Driver Safety Course fees.]

  • Driving Record Request Form
    [Note: This form must mailed to the Texas Department of Public Safety. If you are needing this form as part of the completion requirement for Driver Safety Course, you must request Record Type 3A and pay $10.00 fee. It can take 2-8 weeks for the Texas DPS to return your record.]

  • Deferred Disposition Application Form
    [Note: This form must be notarized and filed with the Richardson Municipal Court on or before your appearance due date. Your payment must accompany this form upon filing with the Court. Do NOT use the on-line Internet payment system for payment of your Deferred Adjudication/Probationary fees.]

  • Statement of Compliance Form
    [Note: This form must be filed with the Richardson Municipal Court at the end of your probationary period.]

  • Plea Form
    [Note: Your decision on what plea to enter is the most important decision you will have to make. Whether you feel that you are guilty or not, we suggest that you read the explanation of each plea provided under the "What to do after receiving a citation".
